What should I bring with me to my appointment?

You should bring copies of all pertinent medical records. This includes doctor or therapy notes, imaging studies, and laboratory test results. In addition, you should bring a photo ID and insurance card.

How early should I arrive to my appointment?

Please arrive 10 minutes prior to your appointment time.

What procedures do you perform?

The physicians at Sea Girt Physical Medicine & Rehabilitation Associates, LLE perform many diagnostic and therapeutic procedures to improve pain and function. Please see the Services section for further details. Currently, the practice does not perform interventional spinal procedures including epidurals, facet injections, or spinal cord stimulator trials.

What should I do if I need a medication refill?

It is most appropriate to refill medications during an office visit. However, if the need arises please call the office to request a medication refill.

Do I need to fill out paperwork prior to my visit?

New patient paperwork can be emailed to you so that it may be filled out prior to your appointment. Otherwise, please arrive early so paperwork can be filled out prior to your appointment time.

What insurance plans do you accept?

We are currently in-network with Medicare (including Aetna Medicare), Horizon, and QualCare. Please call the office to review insurance information with the office.

What is the cancellation/no-show policy?

Patients are expected to cancel appointments with at least 24-hour notice. There will be a $50 fee for all no-shows.

What do I do if I have a billing question?

Please call the office at (732) 974-8100 to discuss any billing-related questions.

What information do I need to provide when scheduling an appointment?

We typically need the patient’s name, date of birth, address, phone number and health insurance information as well as a brief reason for the visit.
